TMD, headaches and Neck pain

TMD: Temporomandibular disorder (TMD) is a condition that causes pain and dysfunction in the jaw joint, osseous components and muscles that control the jaw movements.

TMD is multifactorial. One of the very common structural associations with TMD is the back positioning of the jaw joint.

A back and up positioning of the jaw joint causes pressure and Inflammation in the tissue around the joint.
Inflammation causes degeneration, and damage to the jaw joint.
Disk is easily displaced with the pull from the masticatory muscles.
“Disc-displacement with reduction” When damage is limited to popping and clicking of the jaw and is not painful. 
Disc-displacement Without reduction, too much stress on the disk Can cause perforation of the disk. As the damage progresses disc moves more to the front without going back to place.
Degenerative joint disease (DJD) Damages continue to the point that there will be a bony contact between the condyle and the articular fossa, which will cause more damage and degeneration of the condyle.

TMDs are diagnosed primarily by CBCT. NOT panoramic Xray. 

The posterior positioning or asymmetrical positioning of the TMJ or lower jaw can cause cervical spine misalignments through muscle activities, which in turn can cause disc dislocation and nerve impingement in the spine. This will cause multiple symptoms associated with TMDs, including Migraine headaches and neck and shoulder pain. 

The treatment of TMD’s depending on the severity of the problem and anatomical and structural changes occurred are different. Each patient needs to be evaluated and treated accordingly.
